Chandler Center for the Arts COVID-19 Agreement

I verify that if my child is showing any symptoms of COVID-19 (including cough, shortness of breath or difficulty breathing, fever of 100.4◦F or higher, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell), I will not send my child to camp. I accept, agree, and acknowledge that if my child develops these symptoms during the camp, my child will not be able to participate. I understand that if my child is removed from camp due to a suspected or confirmed case of COVID-19, they may not return to camp until the recommended isolation period has passed, or until they have received a negative test. I agree to inform staff if my child has had potential exposure to someone with COVID-19.

I accept, acknowledge, and understand that staff will to the best of their ability require and enforce all applicable requirements which may assist in offering protection from COVID-19. This includes but is not limited to sharing of objects, cleaning, and hand washing. I understand the atmosphere of the camp may not allow perfect enforcement and assume the risk of my child’s participation. I waive and release from all claims and liabilities of any kind related to that participation, or any requirement imposed on my child as the staff makes their best efforts to comply with guidelines.
