Chandler Center for the Arts Spring Camp 2022 COVID Agreement
I verify that if myself or my child is showing any symptoms of COVID-19, including cough, shortness of breath or difficulty breathing, fever of 100.4◦F or higher, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell at the start of camp, I will not send my child to camp. I accept, agree, and acknowledge that if my child develops these symptoms during the camp or if anyone in the child’s household tests positive for COVID-19, my child will not be able to participate. I understand that if my child is removed from camp due to a suspected or confirmed case of COVID-19, they may not return to camp until the recommended isolation period has passed, or they have received a negative test.
I agree to provide my child with an appropriately fitting face mask covering their nose and mouth for use during the camp which they will wear at all times while at the camp. I understand and acknowledge that my child will not be permitted to participate in the camp if they are not wearing a face mask. I also understand the Center may provide a face mask for my child as a condition to allowing them to continue to participate, and hereby accept the risk and waive and release from all claims and liabilities of any kind related to the use of any face mask provided by the Center.
I accept, acknowledge, and understand that staff will to the best of their ability require and enforce all applicable requirements which may assist in offering protection from COVID-19. This includes but is not limited to social distancing, sharing of objects, cleaning, and hand washing. I understand the atmosphere of the camp may not allow perfect enforcement and assume the risk of my child’s participation. I waive and release from all claims and liabilities of any kind related to that participation or any requirement imposed on my child as the staff makes their best efforts to comply with guidelines.
